A Health Innovation East case study commissioned by NHS England documenting a QI programme that moved physical health check uptake for people with severe mental illness from 10% to 100%. Co-produced with lived experience, delivered through community outreach, and shortlisted for the HSJ Health Inequalities Award from approximately 200 UK submissions in 2024.
